The Cleveland Indians have placed designated hitter Travis Hafner on the 15-day disabled list due to a back injury, reports T.J. Zuppe of ESPNCleveland.com:

To replace Hafner on their roster, the Indians have recalled infielder Jason Donald from Triple-A.
Hafner last appeared for the Indians on August 5 and has only played in one game this month. He also missed the entire month of June due to injury.
The 35-year-old has hit .239/.355/.453 with six doubles, two triples and 11 home runs over 201 at-bats this season.
Donald has appeared in 18 games with the Indians this season, hitting .188/.226/.208 with one extra-base hit over 48 at-bats. The 27-year-old owns a career line of .265/.317/.368 over his three years in the majors.
Donald has also hit .277/.365/.441 with eighteen doubles, three triples and six home runs over 256 at-bats this season.
